Common symptoms include severe fatigue, exercise intolerance or post-exertional malaise (PEM), impaired concentration and memory (\u201cbrain fog\u201d), circulatory problems, shortness of breath, sleep disturbances, and muscular or neurological symptoms.<\/p>\n<p>The underlying causes of Post-COVID syndrome are not yet fully understood. These include autoantibodies against G-protein-coupled receptors (GPCR autoantibodies), inflammatory mediators, and changes in coagulation and endothelial parameters.<\/p>\n<p>This has led to the hypothesis that extracorporeal blood purification procedures might reduce certain circulating factors and thereby potentially influence symptoms.<\/p>\n<p>However, these pathophysiological concepts are still under investigation. Changes in several laboratory parameters were also observed, including GPCR autoantibodies, inflammatory markers, and lipid levels.<\/p>\n<p><small>Korth J. et al. DOI: 10.1055\/a-2445-8593<\/small><\/p>\n<p>However, this was a small, non-randomized and non-placebo-controlled study. DOI: 10.1038\/s41467-025-57198-7<\/small><\/p>\n<p>In 2026, results from the randomized, sham-controlled <strong>IAMPOCO trial<\/strong> investigating immunoadsorption were also published. Forty patients underwent five immunoadsorption sessions and five sham procedures in a crossover design.<\/p>\n<p>Immunoadsorption resulted in a substantial reduction in GPCR autoantibodies. <strong>Despite this biological effect, however, no significant improvement in clinical symptoms was observed compared with sham treatment<\/strong>, including functional impairment, fatigue, Bell score, cognitive performance, and handgrip strength.<\/p>\n<p>Thus, two controlled studies have now demonstrated that removing potentially relevant plasma components or autoantibodies does not necessarily translate into clinical improvement in Post-COVID syndrome.<\/p>\n<\/div><\/section><br \/>\n\n<style type=\"text\/css\" data-created_by=\"avia_inline_auto\" id=\"style-css-av-av_hr-be8425a330a90241e1a8c351bcaeb6a5\">\n#top .hr.hr-invisible.av-av_hr-be8425a330a90241e1a8c351bcaeb6a5{\nmargin-top:-80px;\nheight:1px;\n}\n<\/style>\n<div  class='hr av-av_hr-be8425a330a90241e1a8c351bcaeb6a5 hr-invisible  avia-builder-el-16  el_after_av_textblock  avia-builder-el-last  av-desktop-hide av-medium-hide av-small-hide'><span class='hr-inner '><span class=\"hr-inner-style\"><\/span><\/span><\/div><\/p><\/div>\n<style type=\"text\/css\" data-created_by=\"avia_inline_auto\" id=\"style-css-av-av_one_half-e671fab5613f317ef63eb9ab712c64a3\">\n.flex_column.av-av_one_half-e671fab5613f317ef63eb9ab712c64a3{\n-webkit-border-radius:0px 0px 0px 0px;\n-moz-border-radius:0px 0px 0px 0px;\nborder-radius:0px 0px 0px 0px;\npadding:0px 0px 0px 0px;\n}\n<\/style>\n<div class='flex_column av-av_one_half-e671fab5613f317ef63eb9ab712c64a3 av_one_half  avia-builder-el-17  el_after_av_one_half  avia-builder-el-last  flex_column_div av-zero-column-padding column-top-margin'     ><p><section  class='av_textblock_section av-mtmw7ghe-a3a4cc5ad76a5910164e8c6bc95385f8'  itemscope=\"itemscope\" itemtype=\"https:\/\/schema.org\/CreativeWork\" ><div class='avia_textblock'  itemprop=\"text\" ><p><strong>Current scientific assessment<\/strong><\/p>\n<p>Based on the current state of scientific evidence, the clinical efficacy of apheresis or immunoadsorption for the treatment of Post-COVID syndrome in general <strong>has not been established<\/strong>.<\/p>\n<p>Current German Long\/Post-COVID guidelines emphasize that procedures such as immunoadsorption remain under scientific investigation.<\/p>\n<p>However, the available results do not rule out the possibility that specific, as yet insufficiently characterized subgroups of patients may respond differently to immunomodulatory or extracorporeal treatments. The plasma is subsequently passed through an adsorber that binds immunoglobulins and therefore also removes numerous autoantibodies.<\/p>\n<p>The treated plasma is then returned to the patient.<\/p>\n<p>The IAMPOCO trial demonstrated that this procedure can effectively reduce GPCR autoantibodies. However, these findings have not yet been confirmed by randomized, sham-controlled trials.<\/p>\n<p>We therefore consider further scientific investigation of potential Post-COVID subgroups and biomarkers to be important.
